Clinical Pharmacist: Patient Care & Drug Therapy

SingHealth Community Hospitals is seeking a Pharmacist to provide pharmaceutical care by collaborating with a multidisciplinary health care team to promote wellness and manage acute and chronic illness.

You will recommend and monitor drug therapy, identify drug-related problems, ensure accurate distribution, and educate patients, staff, and other health care practitioners, with regular office hours.
