About the Role

Mater Private Hospital Bundaberg is seeking a highly skilled Clinical Nurse to join our team in a newly created Discharge Planner role. The successful applicant will secure Permanent Full Time employment working a 38-hour work week.

In this pivotal role, you will lead and coordinate the patient discharge process, ensuring safe, timely, and effective transitions from hospital to home or post-acute care settings.

You will work collaboratively within a multidisciplinary team to deliver person-centred care, optimise patient outcomes, and support continuity of care. This is an excellent opportunity for an experienced nurse who is passionate about care coordination, patient advocacy and service improvement.
