Summary of the Position: Case Manager provides intensive individual mental health interventions and support services. Case Manager is supervised by a mental health professional as part of the treatment team.
Assist in the development and implement the interventions developed by the Treatment Teams and describe them in the treatment plan. Provide mental health professionals and other pertinent professionals with clinical information and data to support the treatment plan's objectives as well as engaging participant in an age-appropriate therapeutic activities and provide information and data in a timely manner.
