Case Manager, Mental Health (Toronto)

Role Overview

We are seeking a Case Manager, Mental Health in the Toronto West Integrated Service Team (TWIST) who will complete a comprehensive mental health and physical health assessment of each client’s unique goals and needs through collaborative engagement with each client and their respective support networks.

About the Team

The TWIST is an interdisciplinary program that includes Case Managers with expertise in Acquired Brain Injuries, Mental Health, Mental Health & Justice, Dual Diagnosis, Geriatric Mental Health, Homelessness, Addictions and Nursing.

Primary Responsibilities

• Review referral information of applicants to the program.
• Coordinate and participate in screening interviews with applicants and their referral sources.
• Determine eligibility for the program and confirm acceptance of new clients.
• Develop a collaborative, supportive and therapeutic relationship with each designated client.
