The Neighbourhood Group seeks a dedicated Mobile Shelter Support Program Case Manager to assist vulnerable populations in the GTA. This role emphasizes comprehensive case management within a trauma-informed framework.
As part of the Urban Health and Homelessness team, you will provide essential support to individuals navigating complex issues related to health, housing, and legal services. By conducting assessments and coordinating resources, you will foster an environment of stability and empowerment for clients facing homelessness and related challenges.
