As the Care Transition Coach (CTC) makes essential contributions to the achievement of the Organization’s objectives as a field-based business development representative.

+ The CTC is a hospital based resource for infusion therapy and enteral therapy services offered by the organization.
+ This position educates the hospital staff about home infusion services, increasing awareness and helping identify when home infusion is an optimal solution and appropriate for individual patients.
