Care Team Leader- what's involved in the role?
- The role of the Care Team Leader is to lead the team of Community Care staff who provide direct care to our clients.
- The role ensures that staff have a good understanding of individual client needs and preferences and can provide services tailored to the needs of individual clients, delivered within budget and service expectations.
- The Care Team Leader is responsible for leading, supporting, and mentoring their team, and works in partnership and consultation with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager, where required
