Healthii People is pleased to be supporting Advance Diversity Services in support of recruitment for an experienced Care Partner (Support at Home Packages) to join them on full-time basis. The Bilingual Care Partner will support and work with individuals with their specific needs and goals to maximize their choices and independence. This role is responsible for aligning quality care plans with client-specific needs and effective service delivery through the Consumer Directed Care Model (CDC), within a person-centered, wellness & enablement approach.
