Job Description- Role:  Care Coordinator Location:  On-site – 6019 S 66th E. Ave. Suite 200 Tulsa, OK 74145 Relationship: This position reports to the Director of Operations   Our Care Coordinators help our community by matching our clients and caregivers together.
