Care Coordination Specialist for Home Health & Referrals

Ren Ci Hospital is seeking a capable coordinator to manage referrals from AIC and community partners, ensuring a smooth admission process for home care clients requiring services such as Home Personal Care, Home Nursing, Home Therapy, Home Medical and Medical Escort.

You will handle client data, perform basic financial assessments, conduct home visits where needed, and coordinate internal/external referrals with volunteers.
