**SUMMARY OF POSITION:**

Responsible to provide assistance and support to patients who have no insurance or are underinsured and cannot afford their prescribed medications. Team members collaborate with providers and patients by applying for free drug programs or grants if applicable.

**MINIMUM QUALIFICATIONS** :

+ High school diploma or equivalent.
+ Two (2) years job related experience required.

**PREFERRED QUALIFICATION(S):**

+ Care Coordination, or Patient Financial Counseling experience preferred.
