Assistant Practitioner – Home Based Treatment
To work in HBTT under the line management of the HBTT Team Manager, and accountable to Senior Practitioners, qualified nurses, social workers and allied health professionals for client care programmes. The post holder will work with people of all ages in line with the relevant clinical pathways as outlined within the HBTT operational procedure.
The Assistant Practitioner will function at a higher competency level than that of other community support workers with responsibility for actively participating as a part of the HBTT.
The post holder will be required to play an integral role in the delivery of care as appropriate and will be competent to work across professional disciplines.
Supervision and teaching of junior staff will be part of the role.
To undertake physical health checks for service users under HBTT, including venepuncture, blood pressure, weight, hip/waist measurements, BMI, ECG ...
