Advanced Practice Matron / Advanced Clinical Practitioner

Job summary
The APM/ACP works autonomously and collaboratively to deliver high quality, patient centred care across Leeds City Medical Practice. The postholder provides advanced clinical assessment, diagnosis, treatment and ongoing management for patients with acute, chronic and complex needs primarily (but not exclusively) for patients who reside in residential or nursing homes, or who are housebound.

This is a developing role, and the postholder is expected to shape and evolve the service in line with the needs of the practice, patients and wider health system.

Main duties of the job
Provide advanced clinical assessment, diagnosis and management of acute and long-term conditions using evidence-based practice and national guidance.
