Admission Liaison -LVN - Acute Rehab Unit - Full Time 8 Hours Day (Non-Union, Non-Exempt)

The Admissions Liaison is responsible for collecting and inputting information gathered via detailed patient chart review, communication with patient, communication with patient family and communication with medical team in order to present the data to the Medical admissions team, in order to determine the appropriateness of admission to the acute rehab unit and whether the patient meets acute rehab unit admissions criteria. The Admission liaison is responsible for completing the pre-admissions screen forms, electronically forwarding to the physician, verifying benefits, getting insurance authorization when needed, and insuring CMS compliance throughout the acute rehab admission process This position must collaborate and interact in a positive manner with PPS coordinator, case managers, physicians, discharge planners and other potential referral sources.
