Clinical Pharmacy Specialist – Inpatient Pharmacy Team

We are seeking an exceptional clinically practicing pharmacist to join our Inpatient Pharmacy Team at the Thunder Bay Regional Health Sciences Centre (TBRHSC). The Clinical Pharmacist Specialist (CPS) will be responsible for clinical pharmacy key performance indicators including but not limited to medication reconciliation, developing and initiating pharmaceutical care plans, resolving drug therapy problems, participation in interprofessional patient care rounds and collaboration, patient education, and dispensary related work. CPS will be committed to quality improvement processes, mentorship, education and perform critical decision making regarding medication management. The CPS will report to the Pharmacy Manager.
